Privacy Statement

Glebe Meadow Counselling and Hypnotherapy

In line with the General Data Protection Regulation (GDPR), updated 7 June 2018

 

At Glebe Meadow Counselling and Hypnotherapy, I am committed to providing a confidential, safe, and respectful service. Protecting your privacy is a key part of that commitment. This statement explains what personal information is collected, why it is needed, and how it is securely stored and used.

 

Information I Collect

 

To support you effectively, I gather essential information through the following documents:

 

Client Contact Form

 

Telephone Assessment

 

Contract 

 

Session Notes

 

Closure Form

 

 

The Client Contact Form includes your name, address, date of birth, contact details, GP information, and a unique client reference number. The Contract may also record your next of kin, emergency contact, and power of attorney details if relevant.

 

Other documentation, such as session notes and supervision records, are anonymized using only your client reference number.

 

How Information is Stored

 

All paper records are stored securely in a locked filing cabinet.

 

Any digital communication or data is password-protected and stored securely.

 

Session notes are brief, factual, and used solely to support your therapeutic journey.

 

I attend regular clinical supervision, where aspects of therapy may be discussed. Your identity remains protected throughout.

 

 

Use of Data

 

Anonymized data may be used to monitor the effectiveness of my service 

 

No personal data will ever be shared with third parties unless legally required, or if there is serious concern about your safety or the safety of others, including children and vulnerable adults.

 

 

Your Rights

 

Under GDPR, you have the right to:

 

Request access to the information held about you

 

Ask for any personal data to be corrected or erased (where legally possible)

 

If you wish to make a request, please speak with me directly in the first instance.

 

Retention of Records:

 

All client records are kept securely for a maximum of seven years following the end of therapy, after which they are permanently destroyed. A client may request for their personal data held by me to be destroyed at any time, though this may affect treatment while ongoing.

Ethical Commitment

 

Glebe Meadow Counselling and Hypnotherapy abides by the NCPS Ethical Framework for the Counselling Professions (2021), upholding high standards of professional integrity and confidentiality.
